Questionnaires on Embodia

To use diary questionnaires, you must first set up the questionnaires on your account as outlined in the help article, Creating questionnaires.

Once you've created the questionnaire, from the patient profile, click on Progress > Questionnaires > Diary questionnaires:

Click on the New diary questionnaire button (#1 in the image above). 

In the pop-up form, select the questionnaire that should be completed, as well as the start and end date. During this date range, the patient will have full flexibility to complete new entries for the selected questionnaire.

To view the entries that were completed by the patient for a diary, click on the start date of the diary (#2 in the image above).
