Questionnaires on Embodia

Patients can digitally draw on body diagrams and return them to you digitally! No need for printing, scanning, or emailing.
This feature saves you time, money, trees, looks really cool, and patients love it!

This feature is part of Embodia's Tier 3 membership.

When building a questionnaire, select Question type as Drawing. 

Choose a Background image. Your background image can be:

• Any of our Body charts;
• No background (blank canvas); or 
• You can Upload a new background image.

Create your Question text (#1 in the image below). Make sure your question is Active (#2 in the image below), and optionally Required (#3 in the image below), and click Submit (#4 in the image below).

You can now preview your question by clicking Preview:

Click Exit preview:

You will now find the New questionnaire you made in the Questionnaires tab:

Alternatively, as mentioned above, you can choose No background (blank canvas) as your Background image (#1 in the image below). When choosing this option you can choose to adjust the width and height of your canvas to your desired size. The default size is 800 (width) x 450 (height) pixels. If you desire a smaller blank canvas, you can choose 500 x 250 pixels for example, or any desired dimensions up to 800 x 450 pixels.   

You can now preview your question by clicking Preview. Here is a preview of a 500 x 250-pixel canvas.
